Please fill out and submit this form to the Athletics Office at the beginning of the season.  The Athletic Office communicates base clearance information to DODDs 5 days prior to the event.  All information must be submitted to the Athletic Office at least one week prior to the event

Please be aware that we start a new base clearance list every year so the one filled out for last school year is no longer valid.  Please be sure to fill out your full license plate, not just the last 4 digits.  Each season, families should fill this form out for that season's participating sport(s).

It is advisable to carry some form of photo I.D. with you and please be patient when entering the base.  Often visitors will need to depart their vehicle and sign visitor forms.
